Medicaid Health Management Program

 

Experience HealthND is a health management program of U.S. Care Management (USCM) which has received Disease Management Accreditation from URAC. 

Experience HealthND is sponsored by the North Dakota Department of Human Services, Medical Services Division and USCM, a wholly owned subsidiary of U.S. Preventive Medicine

 FamilyExperience HealthND Registered Nurse Care Managers are available to assist participating Medicaid recipients in managing their health conditions. Depending on your needs, the nurse meets with you face-to-face or contacts you by telephone to discuss your health condition and to assist you in getting the most from your care.

People who have asthma, chronic obstructive pulmonary disease, diabetes or heart failure can help improve their health by following their doctor's treatment plan. Program registered nurses help you and your family by:

  • Assisting you in understanding the treatment plan provided by your doctor
  • Helping you understand the importance of going to your medical appointments
  • Providing up-to-date information on ways for you to manage your condition and stay healthy
  • Helping you find health care services where you live
  • Assisting you with resources needed to manage your health
  • Sharing information with your doctor on how you're doing in the program
  • Working with all of your health care providers to see that your health care needs are met
  • Providing a toll free number you can call for information and support, 24 hours a day, 7 days a week at 1-866-435-4306.
