Experience HealthND is a health management program of U.S. Care Management (USCM) which has received Disease Management Accreditation from URAC. 

Experience HealthND is sponsored by the North Dakota Department of Human Services, Medical Services Division and USCM, a wholly owned subsidiary of U.S. Preventive Medicine

Nurse/Client

Welcome to the Experience HealthND Medicaid Health Management Program!  Utilizing a community-based primary care nurse model, Experience HealthND offers care coordination, education and monitoring services for Medicaid recipients with asthma, diabetes, heart failure, and chronic obstructive pulmonary disease (COPD), as well as their respective co-morbid conditions.   

The Program's goal is to improve adherence to provider treatment plans, build greater self-management skills, and reduce unnecessary utilization and cost of care.  Using national evidence-based practice guidelines as the basis for clinical decision making, R.N. Care Managers work in collaboration with providers who have patients in the Program to extend care coordination and education efforts. 

The Program is working with the University of North Dakota’s School of Medicine and Health Sciences Department of Continuing Medical Education and the school's Library to provide an informational website and free educational programs which will be scheduled for fall 2008.  

A Provider Advisory Committee representing North Dakota physicians and other community health providers has been established to provide advice on issues and needs affecting North Dakota Medicaid recipients with chronic health conditions.

Experience HealthND is offered in partnership with the North Dakota Department of Human Services' Medical Services Division and U.S. Care Management, a subsidiary of U.S. Preventive Medicine.
